Please be aware that it is your responsibility as the insured patient to verify your specific plan for coverage and benefits. You are responsible for understanding your plan limitations, referral, and pre-authorization requirements. Failure to obtain the proper referral or authorization may result in unpaid charges that will become your responsibility. As the patient, you are responsible for all deductibles, co-insurance, and office visit charges rendered by your insurance company. We ask that you provide a valid insurance card and co-payment for services at each visit to Desert Jewel Obstetrics and Gynecology.
